I'm Muskan BDS 3rd year student. March 6 This is an E-log to discuss case scenario of a patient with their guardian's help. I have been given this case to solve in an attempt to understand topic of patient, clinical data analysis to develop my competency in reading and comprehending clinical data including history, clinical findings i,nvestigation and come up with diagnosis and treatment plan . A .60 year old female resident of Nalgonda came to OPD on 4 march  CHEIF COMPLIANT: Dry cough: since 2 days Shortness of breath since 1 day Chest pain 1 day Fever 1day and decrease in urine output.  HISTORY OF PRESENT ILLNESS:  Patient was apparently asymptomatic 2 days ago then she then she started compling of dry cough since 2 days Type of cough : dry cough Onset of cough: insidious with gradually progressing in nature. Chest while cough. Patient also compliance of shortness of breath since 1 day . Grade 3 - shortness of breath on doing less than ordinary work Oliguria - less than 500

A 51 year old male resident of Nalgonda came to OPD on 27 February. CHEIF COMPLIANT: Diabetic foot and pain in his left leg since 20 days because of which he developed gangrene . Fever since 5 days. Throat pain since 2 days  HISTORY OF PRESENT ILLNESS:  Patient was apparently asymptomatic 7 years ago then he developed symptoms of high fever and fatigue for which he had visited local clinic where he was diagnosed with type 2 diabetic milletus . Then 5 years ago he was having fever so he went to regular checkup where he was diagnosed with hypertension.  20 days ago patient had a trauma to

A 53 year old male resident of Nalgonda came OPD on 27 February.  CHIEF COMPLIANT :    Worms in the stool since 1 month   Pain in lower limbs since 15 days because of which is ia       unable to walk .  Pain on touch in upper limbs . HISTORY OF PRESENT ILLNESS:  Patient was apparently asymptomatic since 2 years then he developed symptoms of high fever, fatigue and frequent urination for which he had visited to local clinic where he was diagnosed with diabetic. 16 years old female suffering from seizure

This is an E-log to discuss case scenario of a patient with their guardian's help. I have been given this case to solve in an attempt to understand topic of patient clinical data analysis to develop my competency in reading and comprehending clinical data including history, clinical findings, investigation and come up with diagnosis and treatment plan. A 16 years old female resident of Deverakonda, Telengana came to OPD on 13 February 2023.  CHIEF COMPLIANT: Involuntary movement of both upper and lower limbs since morning and fever since 5 days. HISTORY OF PRESENT ILLNESS:  patient came home 3 days ago with complains of fever and generalised body pains, from hostel for which she has taken to RMP doctor where she was prescribed some medication.after taking that medication her fever subsided but OD observed. Generalised body swelling at around 7pm patient went to pass stool after passing which she started having giddiness . shortness in breath and fearfulness for which she was taken
